Why Hormones Matter: A Guide to BHRT in Perimenopause and Menopause

Perimenopause: Progesterone Goes First

When Estrogen Declines
As perimenopause progresses, estrogen begins to fluctuate and eventually decline as well. This is when the more classically recognized symptoms tend to emerge.
Vasomotor symptoms, or VMS, are among the most common symptoms and include hot flashes and night sweats that disrupt sleep, concentration, and daily comfort. For some women these are mild and occasional. For others they are frequent, intense, and genuinely disruptive to quality of life. While some women do not experience any VMS, most women will experience them to some degree, and they can persist for years.
But estrogen does far more than regulate body temperature. It also plays a role in brain function, supporting memory, focus, and emotional regulation. As estrogen fluctuates, many women notice brain fog, word-finding difficulties, and changes in memory. Mood changes, increased anxiety, and even depressive symptoms are common and are frequently driven by hormonal shifts rather than purely psychological factors.
Estrogen also supports the health of vaginal and urinary tissues. As it declines, many women experience vaginal dryness, discomfort during intimacy, and increased urinary urgency or frequency. Although rarely discussed, these symptoms can significantly affect quality of life and relationships.

While research is still evolving, there is growing evidence that maintaining estrogen levels during the critical early postmenopausal window may support long-term cognitive health and potentially reduce the risk of dementia.
Taken together, these findings reframe BHRT not as a short-term fix for hot flashes, but as a proactive investment in cardiovascular health, skeletal integrity, and cognitive longevity.
